Requesting Medical Records

For your convenience, there are multiple ways you or your representative can access your medical records. Please contact our health information management department for assistance at (305) 348-5238.

Requesting by Email, Mail or Fax

Please follow the steps below when submitting your request by email, mail, or fax.

  1. Download Patient Access Request form. Complete and return the form using one of the options below.
  2. Patient and/or patient representative form (patient access). Click here:
  3. You may email, mail, or fax the completed patient access request form along with a clear copy of a photo ID to:

Please note the patient access request form may be used to request medical records for yourself or to have the requested medical information disclosed to a third-party individual or entity of your choice.
