Please check directly with your insurance plan to learn which behavioral health providers participate with your plan. Each health plan has a different authorization process. Some allow patients to make an appointment without an authorization while others require the patient to initiate the authorization process by calling the health plan or behavioral health network and provide specific information. Please check with your plan or their designated behavioral health network to get specific information.

You are responsible for paying any and all co-payments, co-insurances or deductibles at the time services are rendered. Our fees reflect the training and experience of the physicians and conform to reasonable and customary fees in this community.

Please be aware that there is no co-payment for most preventive visits, however, if our physicians identify and provide treatment for any health problem during a routine preventive visit, you will be responsible for the standard co-payment.

We accept cash, checks, Visa, MasterCard, Discover and debit cards. There will be a $25 fee assessed for checks that are returned unpaid.
